Eligibility & Application
PASSPORT eligibility revolves around two main factors:
- Physical help needed for everyday activities
- Financial help to pay for in-home services
To enroll in PASSPORT or for more information and to determine your eligibility, you must first complete a phone screening to determine preliminary Medicaid eligibility and care needs. Once you are determined to be eligible, a care manager will work with you, your family and your doctor to develop a package of in-home services. The easiest way to get started is to contact the Aging and Disability Resource Center at 877.770.5558.
