Walk-in showers, roofs, water proofing, ramps, stair glides, handrails, HVAC (furnace, hot water tanks, AC units), stair replacement/repairs, grab bars, door widening, flooring replacement, plumbing repairs, electrical repairs, etc.

More Information:

Direction Home has received funding from Summit County to pilot a care managed program called Aging in Place. This program will provide minor home modification (MHM) services to individuals living in Summit County who are aged 60 and over, not eligible for Medicaid Waiver programs and in need of assistance in order to remain safely in the community. Direction Home is seeking MHM providers for the program which will run from January 1, 2024 through December 31, 2025 (with the potential for renewal and program continuation as funding allows). Aging in Place providers will be required to follow Older Americans Act rules and regulations as well as participate in the Unite Us referral platform.

Services offered:

  • Minor Home Modifications, such as walk-in showers, roofs, water proofing, ramps, stair glides, handrails, HVAC (furnace, hot water tanks, AC units), stair replacement/repairs, grab bars, door widening, flooring replacement, plumbing repairs, electrical repairs, etc.

Provider Expectations:

When there is a service need, Direction Home Care Managers send consumer referrals to Aging in Place contracted providers

·        Providers are expected to respond to referrals by either accepting or rejecting the referral

·        Providers are expected to provide quality service in accordance to Older Americans Act rules and regulations for individuals they serve

·        Providers are required to maintain documentation of each unit of service delivered, as well as other records as identified in the regulations

·       Provider will comply with 2 CFR 200, 2 CFR 200.201, 45 CFR 75.352, 45 CFR 75, and 2 CFR 400, as applicable, as well as federal and state law(s).

·       Provider shall permit, at reasonable times and during regular business hours, Direction Home and Summit County to audit its books, practices, policies, invoices and any other documents and records to confirm the Provider’s compliance with the aforementioned laws and regulations.

Service Delivery: See the Ohio Administrative rules, policies, and procedures located in https://codes.ohio.gov/ohio-administrative-code/chapter-173-3. Providers are expected to comply with all applicable regulations and will be audited for compliance. 

Providers have the option of serving the entire county of Summit or a portion thereof depending on the provider’s capacity.

Qualifications:

·        Current contracted providers in good standing

·        Experience in providing minor home modifications

·        Capacity to provide minor home modifications to individuals on the Aging in Place Program

·        Participation in Unite Us referral platform (see www.unitesummitcounty.org for more information and sign-up instructions. It is free to sign up for Unite Us and signing up may also provide additional referrals to your agency outside of Direction Home programs)

Contract Completion Date: Contract period ends December 31, 2025
